1. Medical Services Disclaimer
-All services provided are delivered based on clinical judgment and applicable laws.
-We do not guarantee that any patient will be prescribed testosterone therapy or any other medication.
-Treatment decisions are made solely by licensed healthcare providers based on medical evaluation, laboratory testing, and clinical appropriateness.
2. Eligibility
-Patients must be at least 18 years old and physically located in a state where our providers are licensed to provide care at the time of service.
3. No Guarantee of Results
-Testosterone therapy outcomes vary by individual. The clinic makes no guarantees regarding symptom improvement, fertility outcomes, body composition changes, or other results.
4. Payments & Fees
-All fees are cash-pay and due at time of booking or service unless otherwise stated
-Consultation fees are non-refundable and may credited towards services unless otherwise stated.
-Subscription plans auto-renew unless cancelled per policy and are nonrefundable once paid.
-Patients are responsible for making payments for laboratory tests.
-Failure to make payment may result in suspension of services.
-Prices are subject to change at any time.
5. Refund Policy
-All payments for consultations, medical evaluations, and treatment plans are non-refundable, including in cases where a prescription is not issued.
-Refunds are not provided for partial use of services or dissatisfaction with medical decisions.

-Telehealth services have inherent limitations and may not replace in-person care. Emergency services are not provided. If you are experiencing a medical emergency, call 911 immediately.

-Patients may be required to complete laboratory testing before and during treatment. Failure to complete required labs may result in delay or suspension of treatment.
-Patients are responsible for payment for laboratory tests and associated fees unless stated otherwise.

-By providing payment information, you authorize Men's Health Project to charge your selected payment method for all agreed services, including recurring charges if enrolled in a subscription plan.
13. Chargebacks & Disputes
-Initiating a chargeback without first contacting Men's Health Project may result in immediate termination of services and submission of supporting documentation to the payment processor.
14. Privacy
-Your medical information is protected under HIPAA and applicable federal/state laws. We do not sell or share personal health information except as required for treatment, payment processing, or legal compliance.
15. Modification of Services
-We reserve the right to modify, suspend, or discontinue services at any time without prior notice when clinically or operationally necessary.

-These Terms and Policies are governed by the laws of the state where the patient and providers are located/licensed in respectively, without regard to conflict of law principles.
