Initial Bloodwork $250
Prior to your initial consultation, we require bloodwork to be obtained. This initial bloodwork is essential to determine your eligibility and baseline for starting testosterone optimization therapy and will provide us with a starting reference. We will provide you with a requisition form to be taken with you to a lab location closest to you.
Your lab results will cover:
- Total and free testosterone
- Prostate health
- Liver, kidney, and thyroid function
- Cholesterol levels
- Hematocrit & hemoglobin
- Basic metabolic panel
Testosterone Optimization Therapy
Services include:
- Individualized treatment plan
- Appointments/consultations
- Medications, supplies, & shipping
- Lab reviews
Treatment Plan Billed Quarterly, Biannually, or Yearly
$750 every 3 months
$1400 every 6 months

$2500 every 12 months

1) After booking for your initial bloodwork, you will receive intake forms to complete.
2) After you complete the intake forms you will receive a requisition form to take with you to the nearest partnered lab location to have your bloodwork drawn.
3) After we receive and review your bloodwork, we will schedule an initial consultation to discuss your individualized treatment plan.
